Responsibilities
- Program oversight
- Data analysis
- Plan implementation
- Local systems and processes coordination
- Improves safety culture through measurable improvements
- Identify trends and opportunities for aggregate analysis of reported safety concerns
- Offer recommendations to dyad leader and safety committees
- Promote event reporting and learning from system defects to reduce risk
- Implement policies, procedures, and protocols related to patient safety and clinical risk activities
- Develop and manage education programs on safety culture and reporting
- Review and manage reported safety event files
- Investigate, develop, and facilitate root cause analysis
- Provide consultation on crisis situations
- Support physicians in disclosing unanticipated outcomes
- Collaborate with clinical leadership to minimize harm events
- Serve as subsidiary Patient Safety Officer in absence of director or manager
- Monitor adherence to safety standards
- Participate and present at relevant meetings and committees
- Represent department at committees while maintaining confidentiality
Requirements
- Bachelor’s degree
- Two years of healthcare experience
- Certified Professional in Patient Safety (CPPS), Certified Professional in Healthcare Risk Management (CPHRM), or Certified Professional in Healthcare Quality (CPHQ) or equivalent patient safety training/certification
- If not certified, must obtain within 2 years (full-time) or 3 years (part-time) from date of hire
- Bachelor’s degree in clinical or healthcare focus preferred
- Three years of healthcare experience, specifically in patient safety and/or healthcare risk management preferred
