Program Manager - Care Management

About the role

The Care Management Program Manager reports to the Director of the IFS Department and assists in developing the department’s goals, resources, and initiatives while ensuring service quality, impact measurement, and team well-being. The role oversees the care management program by supervising a team of care managers, facilitating professional development, providing expertise across populations and age groups, and managing program growth. The Program Manager also maintains a small caseload, delivering integrated care management and supportive clinical counseling to IFS clients. Services are provided at all JFCS locations and throughout the Greater Philadelphia area, including remote work, with travel required to client homes, offices, and remote settings.

Responsibilities

  • Coordinate and provide supportive counseling and care management services via home visits, telehealth, and office settings; home visits are conducted to prevent access issues and at client request.
  • Conduct comprehensive client assessments to develop mutually agreed-upon treatment/care plans and enable access to individualized resources (e.g., budgeting, financial issues, public entitlements).
  • Collaborate with community resources (career services, housing) and assist clients with daily life skills to maximize functioning.
  • Maintain knowledge of community resources and refer clients to support treatment goals; provide advocacy with landlords, agencies, family, and natural supports as needed.
  • Prepare and document accurate, timely records including progress notes, assessment tools, and billing information in compliance with department and regulatory standards.
  • Participate in team conferences, staff trainings, and meetings related to care management.
  • Foster an open, engaging environment that improves client service and supports team communication and collaboration.
  • Serve as a resource to the IFS team and lead through open communication, coaching, feedback, and performance management.
  • Conduct regular individual supervisions and case reviews focused on assessment skills, resource use, and documentation.
  • Monitor team productivity, time management, caseload management, and crisis management; ensure accurate and timely documentation.
  • Facilitate team meetings and group supervision to share issues, discuss programs, and communicate agency news and policies.
  • Provide stretch opportunities for employee growth in scope, responsibility, and expertise.
  • Oversee new employee selection, orientation, and training aligned with program policies and agency standards.
  • Create meaningful team evaluations to highlight strengths and areas for improvement.
  • Meet regularly with the Director to plan activities, assess staff performance, and address service needs, team issues, or administrative concerns; recommend solutions and plans.
  • Attend administrative, leadership, and Board Advisory committee meetings as assigned.
  • Recommend initiatives to enhance department activities consistent with the agency strategic plan; participate in strategic discussions on scope of service, program planning, marketing, implementation, and development.
  • Work with the Director to develop and maintain community collaborations and partnerships; represent IFS and the agency with community organizations and professional groups.
  • Monitor program-specific expenses and ensure accurate, timely timekeeping.

Requirements

  • Master’s degree from an accredited graduate school (MSW preferred).
  • 2–4 years of professional experience in a related field (social services, counseling, case management) and 2 years of supervisory experience (may be concurrent).
  • Extensive background, knowledge, and skill in counseling and/or care management.
  • Proficient use of technology (smartphone/tablet/laptop) and experience using MS Office applications.

Additional Requirements

  • Travel throughout the Greater Philadelphia area is required; employee must have access to a car, a valid driver’s license, proof of automobile liability insurance, and a safe driving record on file with the state DMV.
  • Some evening or weekend hours may be required based on program need.
  • Must pass all necessary criminal and/or background checks and meet JFCS vaccination requirements.

  • Participates in annual Trauma Informed Care training.
