Program Implementation SME
About the role
Gritter Francona is seeking a Program Implementation SME to provide senior‑level implementation expertise for the design, launch, management, evaluation, and scaling of an Elevated Risk Care Management (ERCM) Pilot. The pilot aims to improve coordination and effectiveness of suicide‑prevention‑related care for Veterans at elevated risk. The SME will support new projects and programs identified by senior leadership across the full pilot lifecycle—Concept, Explore, Design, Implement, Evaluate, and Spread—whether VHA‑wide or localized to specific VHA program offices. Responsibilities include strategic and operational support for implementation management, performance improvement, strategic performance measurement, programmatic outcomes, resource planning, and evaluation, ensuring alignment with VA‑wide strategic goals, performance targets, budgeting, and evaluation requirements.
Responsibilities
- Program Implementation and Execution: Provide senior‑level subject matter expertise for complex healthcare programs; develop and execute implementation strategies, plans, schedules, milestones, and performance targets; coordinate activities across national, regional, and local stakeholders; support ERCM Pilot implementation at three sites and prepare for phased national expansion; identify barriers, dependencies, resource constraints, and risks, and develop mitigation strategies.
- Elevated Risk Care Management Pilot: Coordinate implementation for Veterans identified through REACH VET, Risk ID, PDE, SPED, post‑event outreach, and Veterans Crisis Line requests; minimize redundant outreach; improve coordination among BHIP teams, MHTCs, suicide‑prevention personnel, and other clinical teams; assess readiness and operational requirements; develop current‑state and future‑state workflows; document lessons learned for scale‑up.
- Strategic Performance Management: Develop and implement performance measures linked to VA‑wide strategic goals; establish targets, indicators, milestones, and outcome measures; monitor performance, create dashboards, reports, scorecards, and executive briefings; analyze trends and recommend improvements; integrate measurement into implementation, evaluation, and continuous improvement.
- PPBE/MFR and Resource Alignment: Apply deep knowledge of Planning, Programming, Budgeting, and Execution (PPBE) and Management and Financial Reporting (MFR) processes; align program objectives, resources, budgeting, and performance outcomes; assist leadership in identifying resource requirements; develop documentation for budget and performance decision‑making; evaluate relationships among activities, resource allocation, and outcomes; prepare executive‑level materials on performance and resource needs.
- Strategic Planning and Program Integration: Translate strategic priorities into executable programs; develop implementation roadmaps, performance frameworks, and operational plans; identify integration opportunities to reduce duplication; coordinate across VHA program offices; ensure alignment with broader VA and VHA goals; facilitate integration of implementation, performance measurement, evaluation, budgeting, and strategic planning.
- Implementation Evaluation and Continuous Improvement: Develop evaluation strategies to measure implementation effectiveness; coordinate with evaluation specialists; capture implementation data for continuous improvement; review performance, identify lessons learned, barriers, and refinement opportunities; produce evidence‑based recommendations for program continuation, modification, or expansion; establish mechanisms for ongoing performance monitoring.
- National Healthcare Program Implementation: Provide expertise for large‑scale integrated healthcare system implementations; develop scalable approaches for phased deployment; identify initial expansion sites based on similarity, readiness, and resources; create transition strategies from pilot to broader operational deployment; support sustainment planning and continuous performance monitoring.
- Stakeholder Coordination and Leadership Support: Work closely with Government program leads, senior leadership, clinical stakeholders, and implementation teams; facilitate working sessions, planning meetings, IPT activities, and leadership reviews; prepare executive briefings, decision documents, implementation plans, performance reports, and recommendations; communicate complex strategic, operational, and financial information clearly; address stakeholder concerns and implementation barriers.
- Technical Documentation and Deliverables: Develop high‑quality implementation plans, roadmaps, performance reports, dashboards, briefing materials, operating procedures, and transition documentation; maintain comprehensive records of activities, decisions, risks, measures, and lessons learned; create standardized tools and templates for consistency across sites; support development of national implementation toolkits and transition materials.
