Program Coordinator (Registered Therapist)
Overview
Key Rehab is seeking a Full-time Program Coordinator-OT or PT to join their team in Sioux City, providing care in Home Health and private practice settings.
Education
- Required: Completion of an accredited PT, OT, or SLP program
- Preferred: Prior experience in long-term care setting and service coordination
- Required: Current PT, OT, or SLP license to practice in the state(s) assigned
- Required: Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
- Required: CPR, as required by community
- Required: Valid driver license and car insurance as required by state law
Licenses/Certificates
- Required: Other items as required by federal, state and local standards
Responsibilities
- Provide therapy under their disciplinary scope of practice (OT, PT, or SLP)
- Serve as the liaison between Key Rehab and community staff
- Minimize administrative burden and provide support to allow clinicians to deliver therapy services
- Cookordination staffing and daily scheduling to meet key performance indicators and expectations under the direction of a Regional Supervisor
