Program Coordinator (Registered Therapist)

Overview

Key Rehab is seeking a Full-time, Program Coordinator-OT or PT to join their team in Coleridge, NE, providing care in a SNF setting. The ideal candidate will have completed an accredited PT, OT, or SLP program and prefer prior experience in long-term care and service coordination.

Education

• Required: Completion of an accredited PT, OT, or SLP program
• Required: Current PT, OT, or SLP license to practice in the state(s) assigned
• Required: Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
• Required: CPR certification
• Required: Valid driver license and car insurance as required by state law

Experience

• Preferred: Prior experience in long-term care and service coordination

Licenses/Certificates

• Required: Current PT, OT, or SLP license to practice in the state(s) assigned

Responsibilities

• Provide therapy under their disciplinary scope of practice (OT, PT, or SLP)
• Serve as the liaison between Key Rehab and community staff
• Minimize administrative burden and provide support to clinicians to deliver therapy services
• Cookordination staffing and daily scheduling to meet key performance indicators and expectations under the direction of a Regional Supervisor
