About the role
As the QIDP Program Coordinator, you will have decision-making responsibility for the overall day-to-day operation of the Day Habilitation Program. You will ensure the program operates consistently with the agency and program-specific mission statements, maintains compliance with all regulations and agency policies, and operates within budgetary limits. Reports to the Program/Site Director of Day Habilitation.
Responsibilities
- Ensures supervision for a team of Supervising Habilitation Specialists, Habilitation Specialists, Assistant Habilitation, and per diem staff.
- Participates in interviews and recommends hiring of new Supervising, Habilitation, Assistants, and per diem staff.
- Provides direct leadership and training to staff on customer satisfaction, program management needs, regulatory compliance standards (including OPWDD, OMIG), and agency policies and procedures.
- Ensures development and implementation of program plans and curriculum development.
- Retrieves, reviews, and maintains annual and semi-annual documents for audit purposes, including Comprehensive Functional Assessments (CFA), Life Plans, semi-annual and monthly reviews, and Level of Care Eligibility Determinations.
- Conducts QIPD reviews of documents, including timely submission of required weekly billing, day habilitation plans, monthly summaries, and all survey-required documents.
- Develops and delivers performance appraisals, manages the progressive disciplinary process, implements performance improvement plans as needed, and maintains associated records.
- Utilizes management tools and databases, including Electronic Health Record systems, to ensure service planning, delivery, referrals, linkages to community-based organizations, follow-up, and collaboration with collateral contacts while maintaining confidentiality.
- Displays proficiency in entering person-served data documentation in the electronic data system.
- Serves as the point of contact for crisis intervention for staff and community providers, evaluating and actively resolving concerns as needed.
- Collaborates with referral sources, including clinical care providers and transportation agencies.
- Represents the agency professionally at all times.
- Develops and ensures implementation of all operational systems, including evacuation procedures.
- Conducts regular staff meetings.
- Ensures the weekly census is completed efficiently and on time.
- Ensures implementation of systems for screening, admissions/discharge, referrals, ISP review, organization, and staffing patterns.
- Monitors participants' progress by observing individuals and staff performing tasks.
- Participates as a member of assigned committees.
- Physically interacts with program individuals, negotiates the environment independently, and assists in all SCIP interventions, emergency CPR, first aid techniques, and fire drills.
- Maintains confidentiality at all times.
- Performs other duties as requested.
Qualifications
- Bachelor’s Degree in the human services field.
- 2-year New York State Driver’s License.
