PRN SUD Case Manager
Position Summary
Delivers quality and effective substance use disorder clinical services to patients presenting and/or admitted to a medical facility for acute alcohol/drug intoxication. Also, assists with review and analysis of utilization data, staff development, peer review, staff training, site visit preparation, and monitoring of clinical procedures and patient outcomes and other duties as defined by supervisor.
Essential Job Functions
- Complete clinical screening, evaluation, and placement of patients.
- Develop placement and discharge plans.
- Collaborate with hospital staff to improve management of patients.
- Cookordinate Substance Use Disorder services provided to Emergency Services Department and additional areas of hospital as needed.
- Participate in planning and successfully implementing clinical strategies to achieve appropriate length of patients stay in acute care settings.
- Plan and implement clinical strategies and resources to achieve increased engagement of acutely intoxicated individuals in indicated treatment services.
- Identify and coordinate the care planning for high utilizers of acute services and develop effective interventions to change default presentation from Emergency Services to identified community-based services.
- Interview patients, review records, and confer with other professionals to evaluate condition of patient.
- Counsel patients to assist them in overcoming alcohol and drug dependency.
- Counsel family members to assist family in dealing with and providing support for patient.
- Counsel family members regarding impact of alcohol and drug dependency in their lives.
- Counsel patients/family members in crisis as needed.
- Refer patients to appropriate services as needed such as detoxification, medical evaluation and treatment, social services, and recovery resources.
- Maintain contact on a regular basis with referral sources.
- Monitor conditions of patients to evaluate success of therapy, and adapt treatment as needed.
- Maintain patient files, utilizing identified technical resources, provide timely documentation in EMR in a site visit ready manner, at all times.
- Develop discharge/transition plans for patients going to SUD treatment.
- Understand and implement all TAP 21 Addiction Counseling Competencies.
- Participate in utilization review activities as required.
- Comply with all policies and procedures relating to clinical licensure.
- Provide training to practicum students.
- Other duties as assigned.

PHI Access Level
This position will have regular access to patient records, but not access to employee medical information, other than his/her own.
Work Environment
On-Site: Office environment with moderate noise level.
