Responsibilities
- Provide physical therapy services to residents in a skilled nursing facility setting.
- Develop and implement individualized treatment plans based on assessment findings.
- Document progress notes and treatment sessions accurately and timely.
- Collaborate with interdisciplinary teams to ensure comprehensive care for residents.
- Participate in staff meetings and training sessions as needed.
Requirements
- Current licensure as a Physical Therapist in the state of Oregon.
- At least two years of recent clinical experience in a skilled nursing facility setting.
- Knowledge of Medicare and Medicaid regulations and reimbursement processes.
- Excellent communication and interpersonal skills.
Qualifications
- Masters degree in Physical Therapy from an accredited program.
- Passing scores on the National Physical Therapy Examination (NPTE).
- Experience with fall prevention and management strategies.
- Ability to work flexible hours including evenings and weekends.
Skills
- Strong clinical skills and knowledge of physical therapy techniques.
- Effective problem-solving and decision-making abilities.
- Proficiency in Microsoft Office Suite.
- Ability to work independently and as part of a team.
