About the Role

The Physical Therapist in Home Health evaluates patients' functional status, mobility, pain, and medical conditions to develop and implement personalized treatment plans.

Responsibilities

• Conduct assessments of patients' needs
• Assist in creating personalized therapy plans

• Monitor patient progress and communicate with the healthcare team, patients, and families
• Complete evaluations, progress notes, and reports on treatment outcomes

Requirements

• Valid state licensure as a Physical Therapist
• CPR certification
