About the Role
Aventura at Carriage Inn is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team. This is a flexible opportunity for a PT who enjoys working with the skilled nursing and long-term care population, focusing on improving residents’ mobility, independence, and quality of life.
Responsibilities
- Evaluate residents and develop individualized physical therapy plans of care
- Provide skilled PT services to help residents improve strength, mobility, balance, and functional independence
- Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team
- Complete timely and accurate documentation
- Participate in discharge planning and resident/family education
- Maintain compliance with facility, state, federal, and professional standards
Requirements
- Graduate of an accredited Physical Therapy program
- Current Ohio Physical Therapist license in good standing
- SNF/LTC experience preferred
- Experience with Net Health and PointClickCare (PCC) preferred
- Strong communication and teamwork skills
