About the role
Aventura at Shiloh Springs is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team. This is a great opportunity for a PT who enjoys working with the skilled nursing and long-term care population and wants the flexibility of a PRN schedule while making a meaningful difference in residents' lives.

Responsibilities
- Evaluate residents and develop individualized physical therapy plans of care
- Provide skilled PT services focused on mobility, strength, balance, safety, and functional independence
- Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team
- Provide resident and family education as appropriate
- Complete timely and accurate documentation
- Support safe discharge planning and continuity of care
Requirements
- Graduate of an accredited Physical Therapy program
- Current Ohio Physical Therapist license in good standing
- SNF/LTC experience preferred
- Experience with Net Health and PointClickCare (PCC) preferred
- Strong communication and teamwork skills
- Dependable, compassionate, and resident-focused
