PRN Patient Experience Associate/Care Coordinator - (Montgomery, AL)

Unlike traditional physician practices, Sage Health physicians manage a patient panel of 500 or less and collaborate with top outcomes-oriented specialists and hospitals. Our model operates under a global capitation budget, allowing us to focus solely on keeping our seniors healthy.

Responsibilities

• Deliver high-quality, patient-centered primary care to seniors within a neighborhood health center.
• Manage a patient panel of 500 or fewer individuals, ensuring personalized and attentive care.
• Collaborate with outcomes-oriented specialists and hospitals to coordinate comprehensive care for patients.
• Operate within a global capitation budget provided by Medicare Advantage plans, focusing on preventive care and health maintenance.
• Contribute to building a new standard-bearing senior care model in the United States.

Qualifications

• Experience in primary care, preferably with a focus on senior or geriatric populations.
• Commitment to patient-centered care and outcomes-driven healthcare delivery.
