Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and SLP, focused on the patient’s plan of care and personal goals
• Assess patient, observe deficits, establish therapy goals and document progress
• Auxiliary patient with exercises to improve fine motor skills and coordination
• Suggest adaptive equipment such as grab bars and shower chairs to offer added support
• Assess fall risks and introduce strategies to improve home safety
• Education patient and family on plan of care, exercises, goals and self-care

Requirements

• Graduate of an accredited Occupational Therapy Program and active OT license in South Carolina
• Minimum of 1 years of occupational therapy experience, ideally in home healthcare
• CPR certification
• Valid car insurance
• Knowledge of state and federal home health regulations

• Maintain accurate and up to date documentation
• Show good clinical judgment, strong interpersonal skills, resourcefulness and compassion
• Provide services in accordance with agency policies and regulations that define scope of practice

Skills

• Strong communication and interpersonal skills
• Ability to assess and treat a variety of physical and cognitive impairments
• Knowledge of home health regulations and standards
