Job Summary

We are currently seeking an APP (FNP, NP, or Adult Gero NP) or PA based in Macomb, IL, serving local skilled nursing facilities within a close-knit, mission-driven care community.

Qualifications

• Advanced Practice Providers must hold an active U.S. license and be board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).
• Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

What You’ll Do

• Provide high-quality, patient-centered care to residents in skilled nursing facilities
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
• Communicate with facility nursing staff via phone or secure messaging as needed
• Maintain reliable on-site attendance for scheduled shifts
• Ensure compliance with clinical standards, facility policies, and regulatory requirements

Schedule

6-hour facility shifts for full-time positions (rounds generally start between 7–10 a.m.), freedom from on-call and overnight requirements, and timely charting using company-provided iPad with LTE.
