About the Role

The Licensed Master Social Worker position enables the facility to identify the medically related social and emotional needs of patients/residents. This role meets those needs through individual and group social services programming, promotes patients'/residents' potential during their stay, and collaborates with physicians and patients/residents in preparation for discharge.

Qualifications

• Licensed Master Social Worker per state requirements or eligible for licensure.
• Master’s degree in Social Work or a human services field.
• At least one year of experience in hospice, long-term care setting, and/or related healthcare setting.
• Skills in communication, documentation, assessment, and social work methods and techniques.
• Ability to read, write, and speak the English language.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ physical, functional, psychosocial, and environmental needs (e.g., risk for grief, cultural and spiritual, verbal and non-verbal).
• Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient eligibility.
• Implement, develop, and document the plan of care to ensure quality and continuity, revising as necessary.
• Consult with and educate patients and families on disease processes, self-care techniques, end-of-life care, and nutrition/dietary needs, prioritizing needs with the Interdisciplinary Group (IDG).
• Provide training to other staff as needed.
• Determine scope and frequency of services based on acuity and patient/family needs, assessing caregiver ability to meet immediate and ongoing needs.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Apply infection control and standard precautions in patient care to prevent contamination and disease transmission.
• Provide clinical direction to Hospice Aides and LPNs/LVNs to ensure quality and continuity of service.
• Prepare clinical and progress notes demonstrating progress toward established goals, ensuring documentation supports continuity of care.
• Inform physicians and personnel of changes in patient needs and outcomes of interventions, evaluating patient and family response to care.
• Monitor assigned cases to ensure compliance with third-party payor requirements.
• Demonstrate commitment to professional growth and competency.
• Promote agency philosophy and administrative policies.
• Provide effective communication to patients, families, team members, and other healthcare professionals.
