PRN Licensed Practical Nurse Needed

About the role

As a Licensed Practical Nurse with Valor Healthcare, you will provide prescribed medical treatment and personal care services to patients with diseases and injuries in our Community Based Outpatient Clinic (CBOC) in Williston, ND. You will collaborate with the core PACT Team (Primary Care Provider, RN, and Medical Assistant) and the expanded PACT Team, including family/caregiver, VA, and community-based services, to develop patient care plans. This position is PRN or as needed, offering a weekday schedule.

Responsibilities

• Actively assist with patient care, physical examinations, and ancillary tests.
• Perform annual and routine nursing assessments and preventive health services authorized by the VA and clinic Medical Director, including:
  • Interviewing and collecting patient information (pain assessment, pulse rate, temperature, height & weight, blood pressure).
  • Recording information in the patient’s electronic medical record.
• Administer medications orally or by subcutaneous/intramuscular injection, and document time and amount in electronic charts.
• Apply hot/cold compresses as indicated and observe patients for adverse reactions.
• Operate diagnostic equipment (EKG, glucometers, pulse oximeters, nebulizers, oxygen equipment, spirometry, etc.) to administer routine tests and treatments.
• Dress wounds and incisions, interpret physician’s instructions to patients, and assist with emergencies.
• Collect lab specimens (urine, blood, sputum) for testing and perform waived laboratory tests.
• Clean treatment rooms, equipment, and supplies using approved wipes/solutions.
• Submit laboratory and radiology results to medical providers and provide private practice results to the VA for scanning.
• Input patient information into VISTA/CPRS and complete clinical reminders due at each visit.
• Assist with front desk duties (check-in, check-out, phone coverage) as needed.
• Support co-managed care collaboration with non-VA providers and facilities.
• Provide patient and family health education focused on self-management, prevention, wellness, and healthy lifestyle choices.
• Advise on community resources and maintain confidentiality of veterans’ data.
• Comply with VA and company training requirements.
• Fulfill compliance requirements (OIG, Joint Commission, Environment of Care, lab compliance).
• Achieve excellent clinical outcomes through VA guidelines.
• Deliver exceptional customer service to veterans, colleagues, and visitors, fostering a welcoming atmosphere.
• Collaborate with clinic leadership to resolve veteran concerns or complaints.
• Participate in outreach events to support clinic enrollment initiatives.
• Support patient enrollment, retention, and follow-up visits.

Requirements

• Diploma or certificate from an accredited practical/vocational nursing program.
• Minimum one year of experience as an LPN/LVN in primary care, ambulatory care, or a government healthcare environment (preferred).
• Must be credentialed and in good standing with the Veterans Health Administration (VA).
• Valid, unrestricted LPN/LVN license in the state of employment or as required by VA contractual obligations.
• Current Basic Life Support (BLS) and ACLS certification (as specified by individual VAMC contract).
• Proficiency in written and spoken English.
• Strong computer skills, including EMR experience.
