(PRN) Hospice Registered Nurse, RN
About the role
Provide care and services for a group of patients, visiting them where they reside—private homes, Skilled Nursing Facilities, and assisted living facilities. As a contributing member of the interdisciplinary team, you’ll offer expertise in psych-social assessments, interventions, and knowledge of community resources.

Responsibilities
- Work within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and aide leadership.
- Provide one-on-one care to patients while receiving strong support from a patient care team.
- Operate independently on a day-to-day basis, handling phone, email, and written communication, meetings with healthcare professionals, and understanding hospice rules and regulations.
- Manage time effectively to handle visits, documentation, and meetings, including scheduling visits, attending required meetings, and responding to urgent needs by prioritizing and triaging multiple concerns.
- Learn and use an electronic medical records system.
- Drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.
Requirements
- Current Registered Nurse (RN) licensure (in good standing) in the state of Ohio.
