Registered Nurse, RN - Hospice - PRN

This is a field-based, one-on-one care role for a nurse who thrives in a patient- and family-centered environment and is called to make a lasting impact through presence, empathy, and clinical excellence.

Responsibilities

• Conduct comprehensive patient and family assessments
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with a focus on pain and symptom management
• Act as case manager, coordinating care across the interdisciplinary team (physicians, social workers, chaplains, and aides)
• Monitor patient condition and adjust care plans to maintain comfort
• Provide education, emotional support, and guidance to patients and families throughout the hospice journey
• Help families navigate complex decisions with empathy and clarity
• Build meaningful, trusting relationships in deeply personal moments
• Maintain accurate, timely documentation
• Participate in IDT (interdisciplinary team) meetings and care planning
• Support quality and compliance standards

Requirements

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Current CPR certification
• Valid driver’s license and reliable transportation

Preferred Qualifications

• Experience in hospice, palliative care, or home health
• Strong pain and symptom management skills
• Case management and interdisciplinary care experience
• Ability to quickly build trust with patients and families
• Experience with Homecare Homebase (HCHB)
