About the role

This is a field-based, one-on-one care role for a nurse who thrives in a patient- and family-centered environment and is called to make a lasting impact through presence, empathy, and clinical excellence.
Responsibilities
- Conduct comprehensive patient and family assessments
- Develop and manage individualized, comfort-focused care plans
- Deliver skilled nursing care with a focus on pain and symptom management
- Act as case manager, coordinating care across the interdisciplinary team (physicians, social workers, chaplains, and aides)
- Monitor patient condition and adjust care plans to maintain comfort
- Provide education, emotional support, and guidance to patients and families throughout the hospice journey
- Help families navigate complex decisions with empathy and clarity
- Build meaningful, trusting relationships in deeply personal moments
- Maintain accurate, timely documentation
- Participate in IDT (interdisciplinary team) meetings and care planning
- Support quality and compliance standards
Requirements
- Active RN license (or compact eligibility)
- Graduate of an accredited nursing program
- Minimum 1 year of clinical nursing experience
- Strong clinical judgment and compassionate care approach
- Ability to manage a caseload independently
- Excellent communication skills in sensitive situations
- Basic EMR proficiency
- Current CPR certification
- Valid driver’s license and reliable transportation
Preferred Qualifications
- Experience in hospice, palliative care, or home health
- Strong pain and symptom management skills
- Case management and interdisciplinary care experience
- Ability to quickly build trust with patients and families
- Experience with Homecare Homebase (HCHB)
