PRN Hospice Chaplain
About the Role
The Chaplain provides personalized spiritual support and counseling to patients and families in various care settings. They collaborate with the interdisciplinary team to assess spiritual needs and document care in alignment with clinical standards. At Lifted Hospice, chaplains offer spiritual presence, peace, compassion, and support when patients and families need it most, serving in patients’ homes, hospitals, or care facilities and connecting them with local clergy or faith communities upon request.
Key Responsibilities
- Assess spiritual needs by listening with compassion to understand each patient’s and family’s beliefs, values, concerns, and spiritual goals.
- Provide spiritual support—including prayer, counseling, and presence—that brings peace, comfort, and meaning while honoring personal faith traditions.
- Participate in interdisciplinary team (IDT) meetings and help shape care plans that reflect the whole person—body, mind, and spirit.
- Document assessments, visits, and interventions clearly and accurately in the clinical record and plan of care, complying with regulatory standards.
- Offer grief and anticipatory loss support to families, helping them process emotions and find hope and spiritual connection.
- Coordinate with community clergy and faith groups, assisting families in accessing outside spiritual resources when requested.
- Maintain up‑to‑date knowledge of hospice spiritual care, participate in continuing education, and support quality‑improvement efforts.

- Master of Divinity (M.Div.) preferred; endorsement or ordination by a recognized faith community strongly encouraged.
- At least one year of experience providing spiritual care in hospice, healthcare, or ministry settings (preferred).
- Ability to offer respectful spiritual care across diverse beliefs and cultures, communicate clearly in vulnerable moments, and make sound decisions under pressure.
- Collaborative and professional demeanor, confident working as part of an interdisciplinary team and documenting care promptly.
- Knowledge of local spiritual resources and willingness to coordinate clergy or faith support when requested.
- Valid Texas driver’s license, reliable transportation, and current auto liability insurance.
Work Environment
The role involves spending most of the time offering spiritual support wherever patients call home—private residences, assisted‑living or memory‑care communities, hospitals, or other care settings. Work takes place both indoors and outdoors, meeting people where they are during one of life’s most meaningful seasons. The chaplain regularly connects with the local interdisciplinary team, including nurses, social workers, physicians, fellow chaplains, and the medical director, to align spiritual care with the overall plan of care.
