PRN Home Health RN - West Cuyahoga County
About the role
Join the Cleveland Clinic team where you will work alongside passionate caregivers and provide patient-first healthcare. As an RN on the Home Care Team, you will deliver the best possible care to patients in the comfort of their own homes within the West of I-77 Service area. In this role, you will provide assessments, complex treatments and education to help avoid readmissions to the acute care setting and ensure comfort.
Schedule
A caregiver in this position works varying shifts, with 48 hours/six-week period, weekend and holiday requirements. Orientation is 8 weeks, Monday to Friday, 8a-5p. Orientation can be flexible to 3 days per week.
Responsibilities
- Provide direct nursing care in accordance with established healthcare organization policies, procedures, and protocols.
- Implement and monitor patient care plans.
- Provide supervision to LPNs, Home Care Aides and families/caregivers involved in the care of the patient.
- Perform accurate and complete admission assessments and reassessments within a specified time frame.
- Assess patient for safety risks, abuse and neglect.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care.
- Assess and document patients' pain using appropriate pain scale and manage patients' pain then.
- Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team.
- Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
- Escalate change in patient condition to patient’s physician as appropriate.
Requirements
- Graduate from an accredited school of Professional Nursing
- Current state licensure as a Registered Nurse (RN)
- Basic Life Support (BLS) Certification through the American Heart Association (AHA)
- One year of clinical experience as an RN in an acute medical/surgical setting
- Six months of experience as a Homecare Nurse
- Valid Driver’s License (VDL) in Ohio and proof of automobile insurance with $100,000/$300,000 coverage
Qualifications
- Bachelor’s of Science in Nursing (BSN)
- Infusion therapy experience

Pay
Minimum hourly: $43.00 Maximum hourly: $43.00
Physical Requirements
Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
