Position Purpose

The Physical Therapist under the supervision of the Director of Therapy evaluates and treats patients recovering from injury, disease, or surgery in a home care environment. They establish treatment plans and deliver physical therapy services to meet prescribed physician orders. They also participate in coordination of services with healthcare teams and educate patients and their caretakers on home exercise plans.

Essential Functions

  • Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
  • Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
  • Provide a written evaluation and plan of care defining physical therapy needs, goals, progress, and expected outcomes.
  • Deliver physical therapy services to comply with prescribed physician orders.
  • Maintain plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determining discharge care plans as appropriate.
  • Collaborate and educate patients and their caretakers on home exercise plans following discharge.
  • Coordinate services with health care team, physicians, parents/guardians, nurses, etc.
  • Document according to policy and procedure, requiring minimal supervision or instruction.
  • Communicate effectively, maintaining minimal technical or clinical denials after payer reviews.
  • Maintain effective communication with staff, healthcare team members, and family.
  • Serve as a resource consultant for clinical staff.
  • Supervise other personnel (home health aides, physical therapy assistants) as applicable.
  • Complete OASIS assessments where allowed by state professional practice and regulation.
  • Perform other duties as assigned.

Knowledge & Experience

  • Degree in Physical Therapy from an accredited university/program.
  • Current, unrestricted state license as a Physical Therapist in the state of Oklahoma.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
  • Current Basic Cardiac Life Support (BCLS) required.
  • 1-2 years recent Home Health experience preferred.
  • Active Oklahoma driver's license.
  • Current vehicle insurance and registration.
  • Required vehicle.
  • Medical knowledge, patience, compassion, kindness, critical thinking skills, knowledge of infection control practices, stamina, and physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, empathy.

Physical Requirements

  • Moderately heavy physical exertion on a regular and recurring basis such as: extensive driving, assisting patient in transfer activities (wheelchair, to bed, to tub, to commode), and providing substantial support to individuals in ambulation.
  • Use hands to finger, to handle or feel, and talk or hear.
  • Stand, walk, stoop, kneel, crouch, or crawl.
  • Sit occasionally.
  • Lift and/or move over 100 pounds occasionally.

Working Conditions

  • General office, patient home setting, exposure to infectious diseases, automobile.

Training will be provided as part of this role to protect the confidentiality, integrity, and availability of PCS' associates and customers' data.
