PRN Home Health Physical Therapist

About the role

New Dimensions Home Health Care is seeking a compassionate and motivated PRN Home Health Physical Therapist to join their growing team in the Fergus Falls area.

Responsibilities

• Evaluate clients and assess physical therapy needs
• Develop and implement individualized treatment plans
• Help clients restore function, mobility, strength, and independence
• Provide therapeutic exercises and rehabilitation services
• Educate clients and caregivers on treatment plans and home exercises
• Maintain accurate and timely documentation
• Collaborate with healthcare providers and care teams to support optimal client outcomes

Requirements

• Current Physical Therapist license required
• Strong clinical assessment and treatment planning skills
• Excellent communication and interpersonal abilities
• Compassionate, patient-focused approach to care
• Able to work independently and manage time effectively
• Home health experience preferred, but not required
