Overview

The Physical Therapist will follow appropriate practices to perform evaluations, timely re-evaluations, and OASIS assessments, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. This position must also adhere to the practices described in the Department Policy and Procedure Manual. The Therapist is required to be flexible with daily scheduling and may be required to work in various patient care settings.

Responsibilities

  • Performs evaluations, determines PT diagnoses, develops a plan of care, and performs physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability.
  • Evaluates and assesses the patient timely, based on principles outlined by practice guidelines, evidence-based guidelines, and regulatory standards (state & organizational policies, requirements of accrediting bodies, etc.).
  • Provides reassessments, including OASIS assessments, at appropriate time points to determine the level of functioning.
  • Updates and revises the plan of care in consultation with the physician and interdisciplinary team members.
  • Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs.
  • Actively participates in multi-disciplinary team meetings, as assigned.
  • Supervises Physical Therapy Assistants and Home Health Aides in providing services, including completion of required supervisory visit documentation.
  • Participates in training and support of all team members, volunteers, and students.
  • Provides therapeutic treatments and evaluation of equipment needs as applicable.
  • Prepares clinical and progress notes that support skilled intervention and assesses outcomes of care.
  • Plans for discharge of the patient.
  • Carries out the treatment plan effectively and modifies it to maximize therapeutic benefit and meet discharge criteria and return to optimal level of care.
  • Utilizes testing results and develops an updated plan of care based on principles outlined in appropriate evidence-based guidelines.

Requirements

  • Bachelor’s Degree in Physical Therapy (Required)
  • Master’s Degree in Physical Therapy (Required)
  • Doctoral Degree in Physical Therapy (Required)
  • 1 year of Physical Therapy experience (Required)

Skills and Abilities

  • Knowledge of the principles of growth and development throughout the life span, ability to assess patients' needs and status by analyzing evaluated data, and interpret information to identify age-specific needs and provide appropriate care accordingly per patient care setting.
  • Demonstrates the knowledge and skills necessary to provide age, goal, and diagnosis-appropriate care.

Licenses and Certifications

  • Physical Therapy (PT) License – Virginia Department of Health Professions (VDHP) (Required upon hire)
  • CPR/BLS Certification – American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required within 30 days)
  • Valid Driver’s License (Required, as the role involves driving a personal vehicle for business 25% or more of the time)
