The Role
You'll work in our skilled nursing facility providing exceptional, personalized care.
Responsibilities
- Optimize the functional abilities and skills of patients by developing a treatment plan that meets the patient's individual needs and improves their quality of life.
- Observe, assess, and document progress in a timely manner.
- Communicate effectively with the therapy team, patient, and family.
- Provide feedback to the designated supervisor on all clinical and rehabilitation issues.
Requirements
- An active state COTA license is required.
- Some SNF experience is required.
