Job Summary

We are seeking a proficient PRN Care Manager to join our team on an as-needed basis. The ideal candidate will be responsible for facilitating patient-centered care and ensuring effective management and coordination of healthcare services. This role requires excellent interpersonal skills, attention to detail, and a passion for advocating for patients' needs.

Key Responsibilities

  • Conduct comprehensive assessments of patients to determine their healthcare needs and develop personalized care plans.
  • Collaborate with healthcare providers, patients, and families to ensure seamless care transitions and continuity of care.
  • Monitor patient progress and adjust care plans as necessary to meet changing health conditions and needs.
  • Cookordinate with interdisciplinary teams to address patient care concerns and improve healthcare outcomes.
  • Serve as a resource for patients and families, providing education and advocacy to enhance patient understanding and engagement in their health.
  • Document patient information, assessments, and care plans accurately and efficiently in electronic health records.
  • Stay updated on healthcare regulations and best practices to ensure compliance and optimal care delivery.

Qualifications

  • Active Registered Nurse (RN) license or equivalent healthcare professional qualification.
