Care Manager PRN

The Care Coordinator is responsible for managing patient care across various stages of illness, including utilization management, transitional care planning, psychosocial assessments, patient advocacy, and education. Qualifications include a minimum of an Associate or Diploma in Nursing, with a preference for a Bachelor's degree. All relevant certifications such as BLS and RN licensure are required.
