PRN/As Needed - Social Work Care Manager

About the role

The Care Manager, as part of the interdisciplinary team, assesses, plans, advocates, and coordinates care from admission to discharge ensuring a safe transition post hospitalization. Duties are performed in alignment with the Mercy mission, values, and service standards.

Responsibilities

• Affirms patient-centered care and promotes a positive patient experience
• Facilitates effective transition planning and length of stay oversight
• Maintains patient experience, safety, and quality of care through performance metrics and best practices

Requirements

• Master's degree in Social Work required or has satisfactorily completed all requirements for the MSW but awaiting conferment by the education facility
• 0-2 years' experience in acute care hospital setting
• Must have a current license in Social Work in the state of employment (LMSW, LCSW)
• BLS (CPR) certification at hire date, required, or within 90 days of hire
• Certification in Case Management preferred
