About the role
Join a leading home healthcare team as a Licensed Practical / Vocational Nurse (LPN/LVN).
Responsibilities
- Provide and document skilled nursing care under RN supervision, following care plans and physician orders.
- Assess patient condition, report changes, and implement appropriate interventions.
- Maintain accurate documentation and update nursing care plans.
- Educate patients and families on disease management and self-care.
- Ensure infection control and compliance with regulations and policies.
Requirements
- Active, unencumbered LPN/LVN license in the practice state.
- Current CPR certification and TB test within the last 12 months.
- Six months of clinical nursing experience preferred.
- Reliable transportation and physical ability to perform patient care tasks.
Skills
- Attention to detail, effective communication, and time management.
- Problem-solving and conflict resolution skills.
- Physical stamina for lifting, repositioning patients, and traveling.
Work Environment
- Ability to function in diverse environments with potential exposure to bodily fluids and infectious diseases.
