Private Duty Nurse LVN - Nights & Weekends

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to deliver skilled nursing care in patients' homes under supervision of a Registered Nurse. Key Responsibilities Include Providing and documenting nursing assessments, actions, and responses in accordance with care plans and physician orders.Monitoring patient conditions, recognizing changes, and reporting to supervisors or physicians.Implementing and updating nursing care plans to achieve optimal clinical outcomes.Educating patients and families on disease management, self-care, and prevention strategies.Ensuring infection control standards and maintaining confidentiality and professional boundaries. Qualifications And Skills Required Active, unrestricted LPN/LVN license in the practicing state, with compact licenses transferred within 90 days if applicable.Current CPR certification and TB test within last 12 months.Preferred six months of hands-on nursing experience; home health experience is a plus.Strong attention to detail, organization, communication, and problem-solving skills.Physical ability to lift 50 pounds, reposition patients, and function in various environments. Additional Requirements Reliable transportation and the ability to travel as needed.Compliance with health-related regulations, including vaccinations and testing, where applicable.
