Responsibilities
- Assessing patient conditions and documenting activities
- Updating care plans
- Educating patients and families on self-care and disease management
- Recognizing changes requiring intervention
- Ensuring compliance with legal, safety, and infection control standards
Qualifications
- Active, unrestricted LPN/LVN license
- Current CPR certification
- TB test
- Reliable transportation
- Preferably six months of clinical experience
- Additional state-specific requirements may apply
