About the role
This role involves providing high-quality home healthcare, ensuring excellent clinical outcomes through compassionate patient care.
Responsibilities
- Delivering personalized nursing care to children and adults in home settings
- Completing electronic documentation via tablets in patients’ homes
- Collaborating with a multidisciplinary team to meet patient needs
- Supporting patient health through adherence to care plans and protocols
Qualifications
- Active, unencumbered LPN/LVN license in the practicing state (compact licenses must be transferred within 90 days)
- Current CPR certification (training available post-hire if needed)
- TB skin test within the last 12 months
- Reliable transportation
- Preferred: Six months of hands-on nursing experience
