Private Duty LVN (Part-Time/Full-Time)

Role: Private Duty LVN providing one-on-one skilled nursing care in patients' homes. Responsibilities Perform assessments, medication administration, ventilator and tracheostomy management, and wound care.Monitor, document, and report patient status closely.Collaborate with families and interdisciplinary teams to implement individualized care plans.Educate caregivers and support patient safety, comfort, and independence. Qualifications And Skills Licensed Vocational Nurse (LVN) license.Compassionate, patient-focused, with excellent communication skills.Ability to work independently and as part of a team.Comfortable with clinical procedures and patient documentation.
