Job Overview
Title: LVN/LPN- Non-Exempt
Job Summary: Under limited supervision, the licensed practical/vocational nurse (LPN/LVN) is responsible for delivering patient care to assigned patients using the nursing processes of assessment, planning, implementation, and evaluation in accordance with federal, state, and local laws and regulations.
Duties and Responsibilities
- Provides skilled nursing as ordered on the plan of treatment and nursing care plan.
- Identifies new problems, changes, and/or resolutions in patient status and communicates them to the appropriate personnel.
- Performs specific treatments and medication administration as per the care plan.
- Functions as a team member under the direction of the registered nurse.
- Communicates shifts according to the plan of treatment.
- Communicates skilled observations and significant changes in patient status to the Director of Clinical Services and/or Clinical Manager.
- Requests modifications to orders or additional orders based on the patient's clinical status and nursing judgment.
- Documents all physician collaboration in the medical record.
- Utilizes available services appropriately and documents their use.
Requirement
- Graduation from an approved certification/program/accredited school/undergraduate/graduate school or post-graduate education to reflect ability to perform job as described.
- Holds a valid/unencumbered verification/license to practice the job as described in the state where services are provided.
- Has professional experience as required per state regulations to perform the job as described.
- Successfully completes assigned screening/competency testing with a proficiency score of 80% or greater.
- Reliable transportation.
- Provides copy of current TB results or Chest x-ray.
- Provides copy of current CPR certification as per agency policy.
- Is flexible, organized, and able to exercise independent and sound judgment to perform job duties/responsibilities.
Physical Requirements
- Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, and reach with hands and arms.
- Visual and hearing capabilities within normal limits, with or without aid.
- Ability to frequently lift and/or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable.
- Ability to perform the anticipated level of physical assistance required to provide safe client care.
Work Environment
- The work environment is highly variable, with care generally provided in private homes.
- Lights, acoustics, air quality, temperature, and environment hazards are not predictable.
- The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
- The employee is required to travel frequently to complete home health visits.
