Responsibilities
- Provide one-on-one skilled nursing care to pediatric and adult patients in their homes
- Conduct assessments, administer medications, and manage trach and vent care
- Perform wound management as needed
- Collaborate with families and care teams to develop and follow individualized care plans
- Document care activities accurately in EMRs
Qualifications & Skills
Compassionate and dedicated with strong clinical skills. Ability to work independently and communicate effectively with patients, families, and healthcare teams. Knowledge of trach, vent care, and wound management preferred.
