Primary Care – Rochester Internal Medicine Associates- LPN 2

Responsibilities

• Remains accountable for provision of direct care to assigned patients.
• Contributes to unit activities and supports enhancement of the care delivery system.
• Works as a team member and assists other staff members with implementing patient care.
• Assumes responsibility of care for assigned patients under the direction of the co-assigned RN or MD.
• Participates in the plan of care, implements appropriate nursing interventions, and contributes to patient/family education through collection of data, interdisciplinary team collaboration, documentation and excellent communication.

Requirements

• Days: Monday-Friday, No Holidays!
• Includes preventative health maintenance, chronic and acute care needs.
• Part of a team comprised of Ambulatory Techs, RNs, Physicians and APP’s.
• Responsibilities include direct patient care, administration of medication/vaccines, minor laboratory testing, communication with patients and providers using EMR.
• At least 6 months in direct nursing care preferred but not required.
