Primary Care LPN

Responsibilities

• Support providers with patient assessments, vital signs, medication administration, immunizations, and wound care.
• Perform triage and patient education.
• Coordinate care and ensure effective communication within a multidisciplinary team.
• Document accurately in the electronic medical record (EMR).
