Primary Care RN or LPN - Mercy Clinic Clayton/Clarkson

Duties and responsibilities

• Partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel.
• Work directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way.
• Support patient care coordination by managing triage calls and messages.

Position Details

• Location: 15945 Clayton Road, Suite 320 Ballwin, MO 63011
• Schedule: Monday–Friday, 8:00 AM – 4:30 PM - No weekends or holidays!
• Must complete a minimum of three months of on-site training prior to transitioning to a hybrid work-from-home schedule.
• After training, this role follows a 50/50 hybrid model, with full-time (1.0 FTE) co-workers expected to work on-site five days per pay period.
• Must also be available to attend monthly on-site clinic meetings.

Qualifications

• Licensed Practical Nurse (LPN): Education - Diploma in Practical Nursing; Licensure - Active LPN license; Experience - 2–3 years preferred.
• Registered Nurse (RN): Education - Associate of Applied Science in Nursing, or BSN, or 4 years of equivalent experience; Licensure - Active RN license; Experience - 2–3 years preferred.
