Duties and responsibilities
- Works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way.
- Pairs with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel.
LPN Qualifications
- Education: Diploma in Practical Nursing
- Licensure: Active LPN license
- Experience: 2-3 years of experience strongly preferred
RN Qualifications
- Education: Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor’s degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor’s degree
- Licensure: Active RN license
- Experience: 2-3 years of experience strongly preferred
Highly Preferred Skills
- Proficiency with Epic EMR system
- Timely and thorough patient triage skills
- Computer competency (most of this position will be computer work)
- Excellent time management and prioritization
- Adaptability in a new and changing environment
