Role Overview
The Primary Care Physician (PCP) provides comprehensive, outcome-focused care for patients, managing up to 450 individuals with acute and chronic conditions. They act as health coaches, guiding patients toward behavioral and health improvements through goal setting and follow-up. The PCP leads a care team, collaborates with specialists and hospital staff, and ensures compliance with clinical guidelines and quality standards.
Responsibilities
- Assess, diagnose, treat, and coordinate care for older adult patients, including those hospitalized or homebound.
- Take full accountability for patient outcomes, engaging with care teams and specialists as needed.
- Lead and collaborate with care team members, participate in clinical and quality improvement meetings, and support center management.
- Promote a positive, accountable, and passionate center culture.
- Recruit patients and providers, and provide excellent patient access and service.
Qualifications & Skills
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related fields; board eligibility or certification preferred.
- Current or able to obtain a State Medical License and DEA registration.
- Basic Life Support (BLS) certification required within 90 days.
- Proficient in evidence-based medicine, lifestyle interventions, and technology tools like Microsoft Office.
- Strong leadership, communication, and influence skills, with a focus on quality care and physician wellness.
- Willing to travel locally and nationally as needed.
