Primary Care Physician, Pod Leader

About the role

This role will drive complex programs from planning through execution with a focus on structure, accountability, and measurable delivery. You will work across business leaders, delivery teams, and strategic partners to align priorities, manage dependencies, and keep initiatives on track. The position offers the opportunity to bring clarity to complex work and deliver outcomes that matter across the organization within a healthcare environment. Location: San Jose, CA - US based candidates only, no visa sponsorship available.

Compensation: $302,764 – $363,654 annually

Responsibilities

• Perform comprehensive in-person assessments partnering with the interdisciplinary team
• Develop personalized care plans for each participant
• Coordinate discharge for various levels of care
• Focus on comprehensive care with an average of 8 patients a day
• Engage in home visits
• Participate in a rotating on-call schedule

Qualifications

• Completion of an ACGME-accredited residency program
• Master's or Doctorate Degree
• Active state physician license without restrictions
• State CDS certification and DEA registration
• CPR and First Aid certification (must be obtained within 180 days of hire)
• Board certification preferred
