Primary Care Physician, Pod Leader

Responsibilities

• Perform in-person assessments and lead interdisciplinary team
• Develop personalized care plans for participants
• Care for various patient placements
• Coordinate discharge for various patient placements
• Focus on comprehensive patient care with an average of 8 patients daily
• Participate in a rotating on-call schedule, available by phone only

Qualifications

• Completion of an ACGME-accredited residency program
• Master's or Doctorate degree in relevant field
• Board Certification preferred
• Active, unrestricted state physician license
• CDS certification and DEA registration
• CPR and First Aid certification within 180 days of hire, preferred upon hire

$280,741 – $330,601 annually
