Responsibilities
- Deliver outpatient primary care to senior members in comfortable, accessible neighborhood clinics.
- Focus on continuity of care and building relationships with patients in a wellness-focused community.
- Address both medical and social needs of patients with a patient-centric approach.
- Lead care teams to provide high-quality primary care, particularly for those with comorbid conditions, the elderly, and/or medically underserved populations.
Requirements
- M.D. or D.O. graduate.
- Board certified/eligible in Internal Medicine or Family Medicine (required).
- Fellowship training in Geriatrics welcome, but not required.
- Active, non-probationary state medical license.
- Highly collaborative with a patient-centric orientation.
- Confident in delivering high-quality primary care, including availability to patients.
- Driven to care for those with comorbid conditions, the elderly, and/or medically underserved.
- Passionate about work and energized by being part of a fast-growing organization.
