Primary Care Physician, FM/IM

Responsibilities

• Provide medical leadership and collaborate with a multidisciplinary team to ensure integrative patient care.
• Diagnose, treat, and manage acute and chronic conditions for a diverse patient population across all ages.
• Offer direct patient care at the clinic and outreach locations within the Tulare County service area.
• Supervise and support physician assistants and nurse practitioners in clinical practice.
• Capture continuity of care for hospitalized patients and those transitioning from medical respite care.
• Coordinate with the EHR team to develop and optimize EHR templates and the patient portal.
• Engage in quality improvement initiatives, ensuring compliance with clinical standards and best practices.
• Participate in community outreach and education, representing Living Water Clinic’s mission and values.
• Attend management meetings and compliance discussions as part of a collaborative leadership team.
• Develop and implement medical policies and procedures, ensuring integration with other healthcare programs.

Requirements

• Medical License: Must hold a valid medical license from the Medical Board of California or the Osteopathic Medical Board of California.
• Board Certification: Board-certified or board-eligible in Family Medicine (FM) or Internal Medicine (IM).
• DEA & Furnishing License: Valid DEA registration and ability to prescribe medications.
• BLS/CPR Certification: Must be current.
• Bilingual in Spanish and English preferred but not required.
• Strong diagnostic, communication, and problem-solving skills.
• A compassionate and patient-centered approach, with a commitment to improving community health.
• Ability to work both independently and collaboratively within a healthcare team.
• Experience with electronic health records (EHR) and medical coding systems is a plus.
