Primary Care Physician

Responsibilities

• Provide comprehensive assessment, diagnosis, treatment, and care coordination for acute and chronic conditions
• Lead and collaborate with care teams
• Act as a health coach to promote behavior change
• Manage patient outcomes and meet performance metrics
• Engage with hospital and transitional care teams
• Ensure compliance with clinical and regulatory standards
• Foster a positive, accountable practice culture

Qualifications & Skills

• MD or DO in Family Medicine, Internal Medicine, Geriatrics, or similar
• Active medical license
• Board eligibility or certification preferred
• DEA license
• BLS certification
• Proficiency in Microsoft Office
• Strong communication and leadership skills
• Availability for local travel
• Fluency in English
• Able to exercise independent judgment and work collaboratively

Pay

Compensation ranges from $214,700 to $306,714
