About the Role
The Primary Care Physician (PCP) provides comprehensive care for older adult patients, managing acute and chronic conditions while emphasizing outcomes, patient engagement, and cost-effectiveness. They lead care teams, coordinate with specialists and transitional care teams, and ensure compliance with quality and regulatory standards.
Responsibilities
- Deliver high-quality, evidence-based medical care, including assessment, diagnosis, treatment, and health promotion.
- Manage a patient panel of up to 450, fostering trust and encouraging behavior change.
- Lead and collaborate with care teams, coordinate with hospitalists and external providers, and participate in quality and risk management programs.
- Engage in patient advocacy, ensure timely follow-up, and promote a positive center culture.
- Support center operations, including recruiting patients and providers when needed.
Requirements
- Strong clinical knowledge in managing complex health needs of older adults.
- Leadership, communication, and influence skills to guide teams and patients.
- Commitment to continuous learning, quality improvement, and evidence-based medicine.
- Proficiency in Microsoft Office and ability to travel locally and nationally for training and recruitment.
- Fluent in English, with independent judgment and a service-oriented mindset.
Pay
This role offers a salary range of $221,141 - $315,915, with potential bonuses.
