About the Role
The Primary Care Physician provides equitable, value-based healthcare to Medicare patients in neighborhood clinics. They focus on delivering high-quality care, preventive medicine, and managing patient outcomes within a collaborative team environment.
Responsibilities
- Assess, diagnose, and prescribe treatments for patients
- Maintain accurate electronic medical records
- Collaborate with leadership and care teams to ensure quality outcomes
- Oversee clinic operations and meet health quality goals
- Commit to delivering exceptional patient experiences
Requirements
- Hold an M.D. or D.O. degree
- Board-certified or board-eligible in Internal Medicine or Family Medicine
- Active, unrestricted medical license and DEA license
- U.S. work authorization
Preferred Qualifications
- Geriatrics fellowship or experience with older populations
- Experience in value-based care and outpatient settings
- Bilingual skills
