Primary Care Physician

Role: Primary Care Physician responsible for delivering equitable, value-based healthcare to Medicare patients at neighborhood clinics. Responsibilities: Assess, diagnose, and manage patient care; prescribe medications; maintain electronic medical records; collaborate with leadership to achieve health quality goals; oversee care team operations and support center functions. Qualifications: Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.), board-certified or eligible in Internal Medicine or Family Medicine, active unrestricted license, DEA license, and US work authorization. Preferred Skills: Geriatrics fellowship, experience with geriatric populations, outpatient primary care, collaborative healthcare experience, bilingual abilities.
