Responsibilities
- Provide hands-on patient care focused on senior needs
- Address social determinants of health impacting patient outcomes
- Consult and collaborate with an established team and patient panel
- Conduct longer patient appointments of 45 minutes each
- Spend 100% of time on direct patient care and related administrative tasks
Qualifications
- Doctor of Medicine or Osteopathic Medicine required
- Board certified or board eligible in Geriatric Medicine, Family Medicine, or Internal Medicine
- Valid, unrestricted state license and DEA license required
- Preferred: 2+ years of experience in primary care
- Preferred: 1+ years managing a Medicare Advantage patient panel
- Preferred: Familiarity with Medicare guidelines and HEDIS quality indicators
