About the role
Primary Care Physician responsible for delivering equitable, value-based healthcare to Medicare patients in a neighborhood clinic setting.
Responsibilities
- Assess, diagnose, and oversee primary care for patients
- Prescribe medications and manage pharmaceutical treatments
- Maintain electronic medical records
- Collaborate with regional leadership to meet quality goals
- Manage and direct the care team with practice managers
- Perform other duties as assigned
Qualifications
- Medical Degree (MD or DO)
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license and DEA license
- US work authorization
Preferred Skills & Experience
- Fellowship training in Geriatrics
- Experience with geriatric populations and value-based care models
- Collaborative healthcare experience and outpatient primary care background
- Bilingual proficiency where applicable
