Primary Care Physician

Responsibilities

• Provide comprehensive primary care services for patients across the lifespan
• Perform physical examinations and health assessments
• Diagnose and treat acute and chronic medical conditions
• Order, interpret, and follow up on laboratory and diagnostic testing
• Prescribe medications and coordinate specialty referrals as needed
• Perform office-based medical procedures within scope of practice
• Document patient encounters and treatment plans within the electronic health record
• Deliver preventive care, wellness counseling, and chronic disease management
• Collaborate with behavioral health and other interdisciplinary care teams
• Participate in community health education and outreach initiatives
• Support quality improvement efforts and evidence-based clinical practices
• Provide follow-up care and continuity of treatment for established patients
• Serve as a clinical resource for support staff and contribute to a positive patient experience

Qualifications

• M.D. or D.O. from an accredited medical school
• Board Eligible or Board Certified in Family Medicine
• Eligible for or currently licensed to practice medicine in Pennsylvania
• Experience caring for patients across all age groups
• Strong commitment to patient-centered care and community health
• Experience in a Federally Qualified Health Center (FQHC) or community health setting is highly preferred
